General Description:


Acknowledging the historical impacts of colonization, the wisdom of Mi’kmaq traditional knowledge and Western medical approaches to health and wellness, the Mi’kmaq Patient Navigator assists Mi’kmaq clients and families with access to healthcare services and provides and/or advocates for supports along care pathways.


As Mi’kmaq Patient Navigator you will be responsible for:

Building relationships of trust with Mi’kmaq clients and families and guiding them through the health system.
Attending appointments, ensuring early identification of client emotional, social and spiritual needs and seeking Western and/or traditional resources and supports to address these.
Coordinating care and assisting with care transitions by serving as liaison between Mi’kmaq clients, their treatment team and Western and traditional health and wellness system supports.
Educating clients/family members on disease, treatment options and follow-up care requirements.
Facilitating communication and cultural understanding with Mi’kmaq clients and families as well as with care providers.
Advocating for improvements in PEI health systems based on identified gaps facing Mi’kmaq clients and families.

Salary will be commensurate with knowledge and experience.



We are seeking an individual with Mi’kmaq cultural knowledge, strong problem-solving and communication skills, and the ability to establish relationships of trust with clients, families and care providers in Mi’kmaq communities and within Health PEI.


Specific requirements

Experience working with and advocating for Mi’kmaq clients, families and communities.
Awareness of Mi’kmaq community and Health PEI health and wellness programs and services.
Degree or diploma in health/social services-related discipline (Nursing, Social Work, Occupational Therapy). Equivalent combination of education and experience in client navigation and/or support will be considered.
Excellent verbal, written and interpersonal communication skills.
Well-organized. Good computer skills and able to keep client files and information accurate/complete.
Possession of a valid driver’s license and access to a vehicle in good repair for work-related travel is required.
